Albany County Long Term Care Consortium
The Albany County Long Term Care Consortium (formerly known as the Real Choices Consortium) is a collaboration of community health and human services providers and governmental agencies whose mission is to provide pertinent information on the many facets of long term care services. It is our goal that by providing this information we will increase the abilities of consumers, caregivers, health and human services providers and governmental agencies to utilize the long term care system to the fullest.
We will accomplish this by educating people in our multi-service systems on issues surrounding long term care, the key being the exchange of information with others on issues and/or problems all of us encounter when dealing each day with the people we serve.
This exchange of information will take place on two fronts:
Conducting a series of seminars dealing with long term care services, i.e., how to access long term care services, and addressing the many barriers that may preclude someone in need from utilizing these services.
These seminars will include lecture presentations from professionals in the field on selected topics of interest.
These sessions also will include an interactive audience participation session with our panel of experts discussing a simulated case that focuses on successfully accessing long term care services.
Cataloguing the information presented here on our website for future reference.
We also will post various “factoids” or points of information, including useful links, relevant to long term care to further educate and assist you.
The targeted audience for this endeavor is consumers, caregivers, health and human services providers, faith-based organizations and governmental agencies from across our multi-service systems.
The Consortium sponsors the Long Term Care Symposium Series covering many topics of interest relating to long term care services. Recent presentations focused on Suicide Prevention and Hoarding Behaviors and Self Neglect.
