| Adult Day Care Program |
Social assistance in a supervised center environment. The program includes planned therapeutic and recreational activities, meals, and transportation.

| Albany County Long Term Care Consortium |
The Consortium provides information on the many facets of long term care to assist consumers, caregivers, health and human services providers and governmental agencies to access services for those in need.

| Congregate Dining Services |
Seniors meet at various sites throughout the county for a hot meal approved by our registered dietitian. Nutrition education is provided.

| Congregate Services Initiative (CSI) |
Services are provided to address the diverse needs and interests of seniors within a congregate setting.

| Expanded In-Home Services to the Elderly Program (EISEP) |
EISEP provides in-home services, case management, non-institutional respite services, and ancillary services such as Personal Emergency Response Systems (PERS) to eligible seniors facing challenges with the activities of daily living.

| Health Insurance Information and Counseling ('HIICAP') |
Counseling, education, and assistance regarding Medicare, (including Part D), New York State EPIC, Long Term Care Insurance, and other health insurance issues.

| HEAP Cooling Component |
A cooling component benefit of $50 is available to eligible households.
|
| |
| Home Delivered Meals |
Meals are delivered to seniors who are unable to prepare meals or who are nutritionally at risk.

| Information and Assistance |
Experienced staff at the Albany County Department for Aging help seniors and their families find the community resources that best address their individual needs and concerns. Call 518-447-7177.

| Legal Counseling and Assistance |
Legal advice and seminars are provided by attorneys knowledgeable in the areas of the law affecting seniors. Representation is available for income-qualified seniors.

| Senior Citizen Identification Card & CDTA Half Fare Card |
Processing and developing of appropriate ID cards for senior discounts and half fare bus passes.

| Senior Community Services Employment Program |
Trained personnel provide assistance in preparing to work and finding work. Paid training is available for income qualified individuals 55 years and older.

| Supportive Services |
Programs include transportation, recreation, adult education, home shopping assistance, case management and preventive health.

| Telephone Reassurance Services and Friendly Visitor Program |
Senior volunteers place frequent telephone calls or makes visits to homebound elderly.

| Weatherization Referral and Packaging Program (WRAP) |
Through the Home Energy Assistance Program (HEAP), eligible low-income seniors are referred to WRAP services to help reduce fuel bills and increase home safety and comfort. Food Stamp eligibility also determined through this program.
