The Department for Aging works closely with its contracted agencies and senior centers [PDF*] throughout Albany County to provide seniors (60+) with high quality programs and services listed below.

Information and Assistance:
Albany County-NYCONNECTS
Experienced staff help seniors and their families find the community resources that best address their individual needs and concerns. 447-7177 is the Point of Entry access line P.O.E. for comprehensive long term care information, referrals, and/or assessments for the elderly, adults and children with disabilities, their family, or service providers. The P.O.E. line is a collaboration between Albany County’s Departments for Social Services and Aging and works to improve or maintain the person’s health and daily function by providing access to affordable medical and non medical services.
Family Caregiver Support Programs
Caregivers services include support groups, individual counseling, caregiver training and respite care to temporarily relieve caregivers.
| Adult Day Care Program |
Social and/or medical assistance in a supervised environment. The program includes planned therapeutic and recreational activities, meals, and transportation. |
| Expanded In-Home Services to the Elderly Program (EISEP) |
EISEP provides in-home services, case management, non-institutional respite services, and ancillary services such as Personal Emergency Response Systems (PERS) to eligible seniors facing challenges with the activities of daily living. |
| Home Delivered Meals |
Meals are delivered to seniors who are unable to prepare meals or who are nutritionally at risk. |
| Congregate Dining |
Seniors meet at various sites throughout the county for a hot meal approved by our registered dietitian. Nutrition education is provided. |
| Nutritional Counseling |
Our registered dietitian meets with seniors and caregivers to provide education and healthy meal choices. |
| Health Maintenance, Socialization and Recreation |
A variety of programs designed to promote social recreation, companionship, healthy lifestyles, and learning experiences. Volunteers are recruited at many of the sites to assist with various programs. |
| Transportation |
Transportation is provided to medical appointments, senior centers, grocery shopping, and group recreational opportunities. |
| In-Home Contact and Support |
Screened senior volunteers provide companionship and support to homebound elderly through telephone calls, scheduled visits, and assistance with shopping. |
| Weatherization Referral and Packaging Program (WRAP) |
Low-income HEAP eligible seniors are referred to WRAP services to help reduce fuel bills and increase home safety and comfort. Food Stamp eligibility is also determined through this program. |
| Senior Community Services Employment |
Trained personnel provide assistance in preparing to work and finding work. Paid training is available for income qualified individuals 55 years and older. |
| Legal Counseling and Assistance |
Legal advice and seminars are provided by attorneys knowledgeable in the areas of the law affecting seniors. Representation is available for low income seniors. |
| Health Insurance Information and Counseling ('HIICAP') |
Counseling, education, and assistance regarding Medicare, and supplemental health insurance and drug plans, including Medicare Part D and NYS EPIC. |
Long Term Care Insurance Education and Outreach Program (LTCIEOP)
Long Term Care insurance covers costs for care at home or a nursing home or assisted living facility. Our LTCIEOP Resource Center provides information on LTC Insurance as well as the NYS Partnership for LTC which will pay for costs related to a chronic or disabling medical condition while protecting your assets.
